Your First Visit
During your first visit you can talk about the problem that brings you into therapy. We can then discuss what therapy might look like, the type of techniques that we might utilize, and review any questions or concerns you have about the therapy process. It may take a couple of sessions to get a complete picture of the problems that you're dealing with.
To save time, you can download the following documents and complete them before your first appointment. First, there is the Initial Visit History Form. This will provide helpful background information. Second, there is the Treatment Agreement Form. You should read this form carefully as this repesents our therapy contract. Sign only when you are comfortable that all your questions have been answered. Finally, there is a Consent for Release of Information Form. This form is necessary if there are other people that you would like to have involved in your care.
